Title:Foster Care Case Manager
Description:The following are some of the job responsibilities: •Provide relevant crisis intervention support, advocacy service and community resources and program referrals. •Maintain a caseload of 12 to 15 foster care cases and/or 30 to 35 resource home care cases. •Complete intake assessments of program participants upon admission into program. •Provide case management and referral services; provide and link clients/biological families to follow-up services. •Develop service plans, progress reports, termination summaries and retrieve all follow-up documentation and/or collect documents from CUA’s. •Serve as a liaison between CUA Case Manager and PLC regarding support activities for the Caregiver, Resource Parent progress with child/youth and overall stability of the home, etc. • Support Resource Parent in their mentoring role with the permanency resource. • Ensure Resource Parent’s compliance with medication administration and documentation. • Support the Resource Parent to communicate with any education, medical, dental, or other service providers working with the children/youth. •Support Resource Parent with developing plan to meet transportation needs of child/youth. •Assist with transportation needs, as necessary. •Review, collect, and file all monthly licensing and other outstanding documents for resource foster parent.
Qualifications:Bachelors Degree
